Your first appointment, step by step

A warm welcome

Take the elevator to Suite 220 on the second floor. Our reception team will greet you — and if Tilly, our service dog, is on duty, expect an especially friendly hello.

Quick paperwork

You'll complete your new patient forms and share your medication list. Reception will help with anything that's unclear, including insurance and CDCP details.

Whether you're here for a checkup, a specialist, or an urgent problem, our team makes sure you see the right clinician — no guesswork on your part.

  • A list of your current medications — including dosages, if you have them
  • Your insurance details or CDCP information, if applicable
  • Photo ID
  • Any recent dental records or X-rays, if you have copies
  • About 15 extra minutes to complete your new patient paperwork

Referred by your dentist? We'll treat the concern you were referred for and send our findings and records back to your original dentist — your ongoing care stays with the practice you know.

Where do I park, and how do I find you?

Parking is free and plentiful right at the building at 8500 Leslie Street, at Leslie and Highway 7. We're in Suite 220 on the second floor — take the elevator up and you'll see us. The clinic, including our bathroom, is fully wheelchair accessible.

What should I bring to my first appointment?

Please bring a list of your current medications, your insurance or CDCP details if you have them, and a piece of photo ID. You'll complete new patient paperwork when you arrive, and our reception team will walk you through it.

How early should I arrive?

Plan to arrive about 15 minutes before your appointment so there's comfortable time to complete your new patient paperwork without cutting into your visit.

I was referred by my dentist — what happens to my records?

We treat the concern you were referred for, and we send our findings and treatment records back to your original dentist, so your ongoing care stays with the practice you know.

Who is Tilly?

Tilly is our service dog and the friendliest member of the welcome committee. Many patients — especially nervous ones — find a visit with Tilly is the best part of coming in. If you'd prefer a dog-free visit due to allergies or comfort, just mention it when booking and we'll happily arrange it.

Do you bill insurance directly?

Yes. We accept all insurance plans and the Canadian Dental Care Plan, bill directly where possible, and give you a clear estimate before any treatment begins.
